Gurus what is last port of entry

I am filling my EAD myself. I came here with F2 Visa in 99.
 After few months I changed my status to H1.
I went to Mexico for passport stamping. In EAD form there are few columns
Date of Last entry----------
Place of Last entry-----------
Manner of Last entry---------------
What date and Status and port I should fill in these columns my home country info or Mexico\'s.

You should write \' Somewhere in the hills between US and Mexico border\'

CRAWLING/JUMPING THE FENCE/RIDING UNDER THE TRAIN/HIDING UNDER THE TRUNCK etc

I\'m just kidding, man. Being in this country for 3 yrs, you should be aware of these things.
The correct answers :

Date of Last entry
If you get your H1 stamped in Mexico during that trip, you should have gotten a new I-94, then THIS IS YOUR DATE OF LAST ENTRY INTO US.

Place of Last entry
Same as above, YOU SHOULD PUT THE US BORDER TOWN WHERE YOU ENTERED FROM MEXICO AFTER STAMPING.
Manner of Last entry
By Land, and if asked \'Were you checked by immigration officer?..you should write \'YES\'. No is for the people who crawled/jumped the fense etc.

Thanks Milk but the problem is when i entered from Mexico they have stamped my passport without issuing a new i94.
