Here are my details

Guys I have a Birth Certificate which was issued in 1999 from Muncipality, looks like that I could get a RFE because of this (somebody mentioned this on these posts ), Guys can anyone suggest me a format for Affidavit of Birth. I want to keep it ready incase I get a RFE.
Does the affidavit needs to be notorized by a notory officer in US ?
 
Help With Birth Certificate

Hi,

I am total novice in this area. I don\'t have a Birth Certificate and under the current circumstances I can\'t get one from the municipal corporation where I was born since it is very remote now. But my lawyer is adament on me getting one. I was wondering there might be atleast one of you out there, who did not have his/her Birth Certificate but managed with an alternative.

Please suggest how can I handle this situation.. yr help is greatly appreciated.
