what is the reason that rfe is issued by INS ?

is it because

a) person has filed insufficient documents
b) INS is somehow aware that he has changed jobs
c) his company has changed names or changed hands or merged and his attorney has filed amended I-140 - and this has been connected to the original application
d) Random sampling
e) Old cases generally get rfe

Gurus - do u feel that the above are valid reasons ?

Insufficient documents and random sampling are the two most reasons for rfe. Other reasons you mentioned usually require diligence by INS staff and based on the # of staff they have, are less likely.
