Today I contacted National Custom Service Center (1-800-375-5283) for applying new AP.

Documents required:

1. I-131 application
2. Copy of the 1-485 receipt
3. Check for $110.00 and payable to Bureau of Citizenship and Immigration Services
4. 2 photos (Side view) and write the name and A# backside of the photo

I am not sure the request letter to be attached.
