Here is a list of documents I filed as part of my green card application, hopefully it would assist some one else in creating their application package. -
I-130 package - filed by sponsor (my wife)
a) Cover letter
b) Check for $355.00, payable to US Department of Homeland Security
c) 1 photo each of applicant and beneficiary
d) Completed Form I130
e) Completed Forms G-325A for applicant and beneficiary
f) Copy of my birth certificate as proof of citizenship
g) Copy of my unexpired US passport as proof of citizenship
h) Evidence of our relationship-
a. Copy of our marriage certificate
b. Copy of our driving licenses showing joint tenancy
c. Copy of our lease showing joint tenancy
d. Copy of auto and rental insurance showing joint tenancy.
e. Bank statement showing co-mingling of financial resources
I-485 package
a) Cover letter
b) Check for $1010.00 for form I-485 ($930.00) and biometrics (80.00), payable to U.S Department of Homeland Security.
c) 2 passport photos of applicant
d) Form I-485
e) Form G-325A for applicant
f) Birth certificate – I could not locate my birth certificate from the concerned authorities of the town I was born in. I am submitting the following documents in lieu of my birth certificate.
a. Non availability certificate from registrar of births and deaths
b. Affidavit from my parents attesting to my date of birth.
c. Copy of my high school graduation certificate confirming my date of birth.
g) Copy of passport page with H1-B visa
h) Copy of “I797A Notice of action” – approval of last H1B
i) Copy of “I797C Notice of action” indicating pending H1B petition.
j) I-693, sealed envelope containing medical examination from civil surgeon
k) I-864EZ, Affidavit of Support, filled out by my sponsor and supporting documents:
a. Copy of federal income tax return (Form 1040, 1099s and W2) for 2009, 2008 and 2007.
b. Copy of pay stubs for the months of May 2010 to October 2010.
l) Form I693 – Report of medical examination and vaccination record
m) Form I864-EZ – Affadivit of support from sponsor.
I-765 package
a) Cover letter
b) 2 passport photos
c) Form I-765
d) Copy of my last EAD (front and back)
e) Copy of driving license
f) Copy of “I797C Notice of action” – receipt notice for last EAD
