Green Card for Mother thro CP

I am planning to apply for my Mother's Green card ASAP. Her visitor visa has been rejected 3 times already so CP is my only option. She is a citizen of India.

My birth certificate has my first name and my mom's first name only and not our full names. I am trying to get an affidavit saying that both are the same person for my mom(I don't know now if it was really needed). Do I have to get one for myself too. I have all the originals ,if I need it too, can I get a notarized copy from here .

Anyone who has applied for green card for parents via CP please send a personal email to me . It will be of great help.

I am planning to do CP for my parents. I submitted I130s. I have same situation with my birth certificate also. Only first name for me and mom. However first name, middle initial and last name for my dad. I used only a copy of this birth cert for I130s. I did not submit any affidavits for my mom or myself for last name. The understanding is that same last name as my dad's applies to mom and myself. I have not heard from USCIS yet.
