GOT my green card when i was 9yr old. Someone help me file the right forms???

I got my green card when I was 9 years old. My dad didn't file for a new card when I turned 14. I am now married and would like to officially change my name to my husbands last name. My green card is also lost, my dad had it but I do not speak to him anymore. Any help would be great
 
Do you have a copy of your green card? If you do and/or know your Alien Registration Number you can file I-90, Application to Replace Permanent Resident Card. It costs $290 +$80 for fingerprinting.
