I am doing CP and I will have an interview on the first week of May. I just got the second package last week. The information in the package explains the document I need to bring for the interview. It said I need to go for medical examination as described in the Instructions for Medical Examination with the list of hospital name. However, there are 3 forms missing;Optional Form 230 II, Supplemental Information Letter, and IRS Form 9003.
Should I contact the U.S embassy for these document? or should I just download them from the internet? Does Anyone receiving the 2nd package recently have the same missing document as me? and What did you do? I don't know I should download the Optional Form 230 II or not as I think the counseller might have it from the 1st package I sent back to KCC.

Yeah, I also think I don't need to bring any to the counseller. They should have the Optional 230 form in hand now from KCC. How about INS 9003. I think I need to fill in or something. By the way, in the letter, it mentions that any application receiving a visa must travel to the U.S within 4 months. I think I saw someone said 6 months. Is the regulation changed?
