Got 1st FP Notice - Have a question

After a long wait myself and my wife got our first FP notice yesterday by mail. Our appoinment has been scheduled for Jan 07, 2005 at Ft. Lauderdale, FL.

I have got a question. My wife has left to india last week and will be back only on Feb 20,2005. What do I do to re-schedule her appoinment after Feb 20th. Can I call then and ask for a schedule change.. is it possible ??

In the FP notice itself there will be a date, usually 30 days from appt date, within which you can go and give FP in case you miss the appt date. I would suggest you ask your wife come before that date and give FP
 
You go for FP

for her call and reschedule FP giving them reason. if in written it would be good to. during ur FP remind them too about why she is unable to appear for FP. if u submit any signed letter from her that would be good.

I would reschedule the FP for your wife. Then they will send new FP notice.
Once your wife comes back, if new notice has not arrived, take the old notice and try to get the FP done...they will do it without any problems withing 60 days of the notice date.
 
I also believe you have to make a reschedule selection on the original notice (should be check boxes about 2/3 of the way down the notice) and mail it to the ASC for rescheduling. At least in Atlanta, that's what my form said to do. I did that and attached a cover letter explaining that my son had exams that day that he could not miss.

PS. Yes, I was nervous sending them the original notice, but that's what it said to do.

Is there any format (covering letter) for appoinment rescheduling that any one of you used??...

Should I have to send my wife's original FP receipt for rescheduling?. If so, How can we take the FP receipt to the ASC (after wife's arrival) if we haven't received the new rescheduled FP.

Pls let me know..
