Passport
I 797 (h1 approval)
I 129 (h1 petition)
LCA (Photocopy)
2 Photos
Degree/Diploma (Photocopies)
Marrige Certificate (Photocopy) for H4
Fees (see website)
Letter from Employer
Paystubs
Bank statement
