Thanks for your concern buddy, but they are just dummy names and dummy numbers. But is the format of this letter okay?
 
before I give any suggestions,,,I would also need your date of birth and mother maiden name :)

ok now we all got the info and have opened several credit cards:-)

AC21 letter is not essential unless your 140 is being revoked or you get the NOID. Actually by giving that letter you make sure you get the interview. if you were on future GC basis and didn't work for the sponsoring GC then AC21 letter is helpful and the best time to send in any case is if you see an LUD on your 140

Do not include SSN. Include your A# and I-485 receipt number on the subject line it self

One more question:
I do not have the copy of my approved 140 notice. So can I send the following for my AC21:
- Copy of I-140 filing receipt notice
- Copy of USCIS online case status of approved I-140

Just I-485 receipt notice may be enough. Do not not I-140 receipt notice which may create some other confusion.
