If it's over a year and you have not yet received your green card, you may want to do the following:
Contact the INS Service Center that adjudicated your case and inform them that you did not receive the phsyical green card. Make sure they have your correct address and let them know how long it's been that you have not received your card. Ask them to see if they've received your I-89 form and if not ask them to send a request to you to have that take again at the local INS.
Why they should send out a request, is because once the I-89 is taken, the local INS can attached the request and send it back to them and hopefully it won't get lost and go straight to the person who requested and he/she can make sure that the card gets prints.
The main point is to make sure the INS is aware that you have not received your card and to make sure they have your current and correct address.
