Getting an emergency AP

Had applied for spouse's AP at VSC center (RD July 31 2003) and is a derivative applicant.

My MIL has now taken ill and my spouse wants to visit. I am planning to go to Philly local office to try get one. What documents do I take with me apart from the standard ones like passport, I-485 receipt notice, marriage certificate.

1) Is a Medical certificate from doctor attending the sick patient enough, or supporting reports also required.

2) Do I need to take 2 ADIT photos?

3) Do I need to pay them $110 again or will the receipt notice of AP work?

Any experiences/suggestions from fellow members in general, particularly from Philly, would be appreciated.

Thanks

I went last week in my local office and officer told me that I need to support how much serious is the nature of illness.? what is actual illness .? this is to certified by doctor. Also birth certificate to prove your relationship with the ill person. Further officer said if I do not have this then need not come to local office.
