G-325a

I have a question regarding filling the G- 325a form for myself.i have been living in USA for 3 years, only 2 months of them i ve been working.(2 jobs for one month only)i dont even remember the company where i worked (i know it sounds stupid but thats a sad story for me ) of course i never files taxes cos had a tourist visa and was sponsored by my fiance.so what i should write in this case about my job? i dont want to lie and dont know how to check my previous work. please help me with any advice
