G 325A question

When I downloaded the G-325A form there are 4 pages. As I understand one has to be filled by me and another by my husband who is a US citizen. What are the other 2 pages for? I am confused.

khushibose, the G-325A form has 4 pages.
Both of you will have to fill out all four pages.
4 pages for you, and 4 pages for your husband.
