G-325A question

On line 4 of the G-325A where is asks for info on my spouse, it asks for family name (maiden name for wife) but there are TWO sections that can be typed in on the .pdf that can be filled out.

Does this mean I put in ONLY her maiden name, or her legitimate legal surname AND her maiden name?

Each person will fill out one G-325a.

On yours, you put in "Wife" on the 1st Section and then "Wife's Maiden Name".
On hers, you put "Husband" on the 1st Section and then "Your name here".
 
Gah! I read this JUST too late. I just sent papers off and put only her maiden name in there, I did not write "wife" nor did she write "husband" in for mine.

Ah well, we'll see how picky they are in a couple weeks I suppose. Strangely I feel OK about it all.

sorry, but this is silly. if you checked off 'male' on top of form, the name in that section is necessarily your wife's, since gay marriage is not accepted anyway for immigration purposes...
