Hi guys,

The PDF file for form G-325A, there are more than one page but each page is identical. Do I need to print out all the pages and submit them all or just one page is enough? Thanks

All four pages. The PDF form should fill them all in for you at the same time.

That is correct, fill them out using acrobat reader on your computer, you fill out the first page and the software fills out the rest.

Then just print it out and submit all 4 pages.

The pages go to different areas potentially (based on the codes on the bottom left), so make sure all four pages are signed.
