G-325a form questions

I have a question about G-325a that me and my wife who is USC supposed to fill out for form I-130 attachment

1. At the bottom it says this application is filed in connection with.. should we both check "Status as permanent residence?"

2. There's a big box at the bottom that says: Print your name and Alien Registration Number in the box outlined by .... whose names go in there? me or her?

Answer 1. Check Status as permanent residence.

Answer 2. All Marriage to a USC cases, there are Two G-325a's that need to be filled, one for you and one for your Petitioning Spouse, The one that is for you is where you will print your name and a A number if you have one previously.
On your Wife's G-325a she will put her A number if she is a Naturalized USC.
