FP and Blue form

Did anybody get a blue form instead of a yellow form alongwith the FP notice from NSC?

What are they asking in the blue form? Are they asking you to send a letter from your employer and you ead cards copy? If yes, then it is same as yellow form. If not, please post here what is it?

The blue form has the exact contents of the yellow form, I checked up with a friend of mine. I wonder what the significance of the color is!

Yes. I agree. the one we got from INS was yellow and the one we got from authorney to let us know the FP dates etc. had exact same content & purpose. so, I guess may be somebody used yellow or blue for making copies whichever came handy.
