Do I need to get anything else beside the I-693 form from the Civil Surgeon ?
Did anybody have to carry any other documents to the Civil Surgeon's office besides the IDs and the vaccination records and stuff ? The staff at the doctor's office say that I need to get documents from my attorney but they don't tell which damn docs ?
I found the I-693 at http://uscis.gov/text/formsfee/forms/i-693.htm, but on the top right corner of that form it says :
"OM No 1615-0033 Expires 04/30/05"
Any ideas what this expiration date means ? Am I looking at the right and latest place for I-693, if not, please guide ?
