Format for response to RFE - Birth Certificate

Guys,
Just wanted to know what is required to respond to an RFE on a birth certificate. Like several others I submitted a birth certificate made 7 years back, together with my I-485 application. From the threads here, seems like there is a strong possibility of getting an RFE in such situations.

Is there a standardized format to an RFE response. I recall someting about an affidavit. Also - whose signature would be required in such cases. Would it involve sending a family member to my place of birth ( no family members currently living in my place of birth ).

Just wanted to be prepared in advance IF I get an RFE on a birth certificate in the coming months.

when you receive an rfe, it will give you details in ways you can present proof of birth, including hospital records, affidavit from immediate family members (notarized), religous and school records etc. the rfe will even tell you what are your relatives attesting to. hence you don't even have to come up with the wording. also, your lawyer will be able to assist you in preparing the statement at no extra charge.

I only sent two affidavits from two uncles after i received my rfe and got approved. didn't show school, hospital or any other records. the reason why the birth certificate was rejected was because it was generated too late after my birth.

I haev a question. There was a birth certificate for my wife, but that got retained in school. So when we tried to get a non-availability cert, they instead issued a duplicate birth certificate as they already had the certificate on their records. Even in such cases, do we need to obtain non-availability certs and affidavits?

BCIS/INS Birth Certificate requirements

For Linuxrox ---> You cannot get a non-availabilty as the birth has been registered, use the duplicate Birth certificate and have 2 affidavits ready if needed.

INS BIRTH CERTIFICATE REQUIREMENTS

The long form birth certificate refers to a birth certificate that
contains both parents full names and the childs name. If you do not have the long form birth certificate, or do not have a birth certificate at all, you/each applicant will need 2 notarized affidavits which will support the facts surrounding
the birth facts.

FOR APPLICANTS THAT HAVE A SHORT FORM BIRTH CERTIFICATE

You will need affidavits (one from each parent). Please have each parent complete and have notarized.

FOR APPLICANTS THAT DO NOT HAVE A BIRTH CERTIFICATE AT ALL

Each applicant will need the two signed notarized affidavits (one from each parent). In addition each applicant will also have to contact a local competent government agency in your home country and have them give you a notarized affidavit/letter stating that a birth certificate does not exist in your name. This certificate is called a "non-availability certificate", and again, it also must be notarized by the government agency issuing it.

There are no exceptions or substitutions for these INS birth certificate requirement.
