Format for replying RFE

I have got an RFE regrading name mismatch in my passpost/visa and 485 application. Thay have asked me to give explanation for this. So my question is , in what format should i explain? any idea? (is it in letter format or question & answer format or any other way?)

I already asked my attorney. But i haven't got a reply yet. So i thought i'll be prepared if i didn't get an answer from him.
