Format for non-availability of Birth certificate???

Can anyone post the format of the Non-availability of birth certificate?
Should the parents also sign the document or just notarisation is sufficient?
I was born in Vizag, no birth certificate...got 2 affidavits from Chennai as my parents live there...
Can I get the non-avalability cert from Chennai ???
Please resond ASAP...

It is a standard format.

Non-availability certificate is a standard form from your local municipality. There is no standard format, and certificate itself might be different from municipality to municipality.All you need is a complete certificate notorization not required.
