Here is one format...

I, (Please indicate Mother’s first and maiden names for mother’s affidavit. For father’s affidavit, please indicate first and last names.) being duly sworn, hereby affirm and state:
1. I reside at (current home address).

Also, has this format worked before? Is it approved by INS?
I am completely ignorant about these things, so please excuse my stupid questions
Also, does this have to be accompanied by a \'Non-Availability certificate\'. Who gives this? What happens if I donot have a \'non-availability\' but just have the affidavits from parents?
