Form I-693 for medical URGENT

Hello

I have got done my medical exam required for adjust of status.
My doc gave me only a single sheet of page in a sealed envelop to send to the USCIS for the purpose.
My QUESTION is, is it only one page to be submitted in the sealed envelop, as the form doesnt have any of my passport details or reciept numbers for the form 140's/485

He has only given me page 3 of the attached file, is that sufficient?

Please advice as i have recieved an RFE for this in January and have to submit it in a timely manner.

You should be fine, My Dr also send only 1 page (original) and may be few copy of same page, it was page 1 for me and I guess page 7 for my wife,
I asked him to send all pages as original and he said that they do it everyday and USCIS accepts it.

Passpost number is written on Supplemental Form to - I-693, which has immunization history and it should also go with the I-693, check if this was sent then I guess you should be fine

I think supplemental form is absolute necessity, that is what my attorney told me.

USCIS rejecting more I-693 now???

As for supplement to I-693 (vaccinations), did you all have to get three tetanus shots? I had my tetanus shot last week, but the doctor just told me yesterday that I would have to take two more shots before he could filled out the form. That means that I have to wait 7 more months (1 month for the second shot and 6 months for the third shot) before filing my I-485.

The doctor said that USCIS had been returning many forms which he had filled out because they were "not appropriately completed.". I wonder if USCIS is now making it more difficult for doctors to fill out the I-693 so that many of us who got our Labor Certification recently will not be able to file I-485 before the application fee increases to up to $905 as soon as this April.

I would like to hear about others' experiences.
