form I-485 evidence

For the Form I-485:

It says about the medical examination and criminal history, do I have to go to do those first and then attach the evidence with the form I-485? If so how to do that?

Also, on the form for the question:
Place of Last Entry into the US.

I was flying into Atlanta before transitting to my designated city that is not international airport... what should I put there? Should I put atlanta or the city that I live in?
