Form DS 157 for revalidation

hi prashant,

do you know where in visa-services.com are listed the entire list of documents you need to enclose for your drop box application?

From when the form ds-157 is applicable. i sent my
passport for revalidation on jan 07,2002 and received at
st.louis on jan 08,2002.
 
Effective Immediately !!!

That is all what the DOS web-site says - "Effective Immediately", without any date on their web pages. However, I have heard dates ranging from 11th Jan (from my lawyer) to 14th Jan (Delhi Embassy web-site).

Even though you had sent your application before these dates, you might want to contact the folks at St. Louis, and ask them if you do indeed need to send the DS-157 form.

Can you tell me if it is ok to print the form only on one sided paper

Or is it required to print it on double sided paper

what about other docs

i had already visited that link but i dont think it gives the entire list of documents
it says: passport, visa forms, photograph, demanddraft, i797 and that\'s all.
what about degree certificates, tax returns, photos of your workplace, stuff like that??

have the following questions completing the DS-157 form. Didnt get any response from my lawyer for these questions....

3. Full Name (In Native Alphabet)


9. List all the countries u have entered in the last ten years.


12. Not including current employer, list your last two employers


18. Have You Made Specific Travel Arrangements ?

3. Yes, write it in your mother tongue alphabet.

9. You might not need to fill in India, as you are an Indian citizen.

12. You should fill in your University details, as it was your previous employer.

18. If you have already booked your return ticket, then fill in the itinerary of your return trip to SFO, i.e., flight numbers and date/time of travelling.
