Forgot to key-in the last EAD approved date and submitted

Hi,

Forgot to key-in the last EAD approved date on the form and submitted (paper file) it.

What option I have to fix the problem ?

I have attached the copy of my EAD.

Which date should I key-in ?

Recept date or Notice date or approved date (i.e last year start date of the EAD) ?

Which USCIS Office? and date ? Service Center that approved your last EAD (i.e. Texas, Nebraska etc)
Which date should I key-in ? Last approved Date - Notice Date on Approval Document (NOT EAD CARD)

I am filing for my EAD renewal. I do not have a approval notice for my previous EAD, it was probably sent to my lawyer.
I directly got the approved EAD card in mail. Can I write that date for question 11 - which USCIS center ? What date?
Please help.

After I have applied for the EAD first time last year, I got "I-797C, Notice of Action" from USICS first and later I got the "EAD mailer with my EAD card". When you say "notice date on Approval Document (NOT EAD CARD)", do you mean ""I-797C, Notice of Action" or "EAD mailer with my EAD card". I don't see Last approved Date - Notice Date on Approval Document on the "EAD mailer". But I see "Notice date" on "I-797C, Notice of Action".

As for EAD, I have received ""I-797C, Notice of Action" with Notice Type: Receipt Notice" AND "EAD Mailer with EAD card".

I have not received "I-797C, Notice of Action" with Notice Type: Approval Notice" .

Which date should I key-in ?

I had no clue about it either UNTILL my lawyer sent me that few months ago.
Ask your lawyer to send that I-797.
