I have received some PM's recently asking for FOIPA format and I am attaching the format as I am not able to send it through PM's as it is exceeding the limit of characters. I have just written a letter with the below format to Mr. David Hardy's address and I have received the response in like 3 weeks and I did not notorize or pay any fees for FOIPA request.
Please use your judgement to use this format as I am not an expert on immigration matters and if in case, you have any issues, I am not responsible.
