I am planning to use FOIA to get my LC and I-140 copies. I have some questions here. I would appreciate if any body can answer these questions.
6. Data NEEDED on SUBJECT of Record: (If data marked with asterisk (*) is not provided records may not be located)
• Petition or Claim Receipt #:
Question1) Do we need to write I-140 recipt number or I-485 receipt number?.
Question2) do we need to notarize this or just can we take a oath? I read the instruction it is obscure to me. Any help greatly appreciated.

Form 361, Certification of Identity, may be used for this
purposes).
