Finger Printing Question

Hi Gurus,
I am planning to get finger prints taken from Identix Fingerprint Services.They said that they will give "Finger Print card" along with the finger print.

But Canada immigration website says that we need to get "Finger Print Form" from FBI .

My question: Is the FingerPrint card and FingerPrint Form are the same or different.

i took mine in the finger print card....i think both are same.. we fill the form which is same as the card and take fingerprint on it....
