Filling up I-765 form for L-2 visa holder?

We are in the process of filing form I-765 for my wife who is on L2 visa.

We are not sure what should be filled in for item numbers 14 & 15 in the form?

14. Manner of last entry (visitor, student etc)?
She came here on L2 but what is the right term to fill in here??

15. Current Immigration Status (Visitor, Student etc)?
What goes in here??

We are not sure what will be the impact of answering these questions incorrectly.
Please help.

Answer to both the questions would be L-2.

Btw, where do you get the picture, as per INS requirement, from ?
