Filling out I-90 for name change

Hi: I have to fill I-90 form to change my name on green card. The form does not give you the option of current name and new name. Which name and signature should i use on the form; one that is currently on the green card or the new name? Thanks BUUSA

could u elaborate plz,did u have a name change through court?

Hi, Yes i have the court order and have my name updated on the SSN card and driver's license etc.... Now i have to udate the information on my green card. But snce the I-90 from does not have the option of current name on the green card and the new name ... i am not sure which name to put on the form; the one that i currently have on the green card or the new name?

Right the new name everywhere , same thing happened with a friend. He wrote the CORRECT AND CURRENT NAME while filing.
