Filling form G731 for CSC

I need to inquire on my plastic card status.. my questions regarding the form are

1. Should I fill out the first page or the second page? My guess is page 1 - is that correct..

2. If I should fill out the first page what does "Date entry of adjustment mean?". Also should I leave page 2 empty or rather even send only page 1?
