I have a question about filling column "Have you ever been issued a US Visa" and "enter additional visas" ( column 30), do I type in the dates on which I got my passport stamped with the visas or do I type in the start dates of my I797s?

I-797 is not a visa, its just a petition approved for visa. You'll have to enter the dates of actual visa stampings (if any) on your passport and type of visa H1/F1 etc...
