Filling DS-156,157,158 forms for H1B Revalidation

Hi,

I got my visa extension recently and I am planning to revelidate the I-94 upon re-entry.
I planning 2 things:
1) Getting it stamped from Canada on Feb 7th(Ottawa)
2) Getting it stamped from India as a back-up option(Chennai)

I need to fill my DS-156 form in both the above cases when I apply for an appointment in Canada and Chennai India.

What would I need to fill in the following questions for Chennia?Is it my India Address or US Address?

Q15: Home Address
Q16: Home Telphone


What would I need to fill in the following questions for Canada?do I need to fill my US Address?If I need to fill my US address then for Chennai if I fill my Indian address would it become a mismatch for the applicant having diffrent addresses?

Q15: Home Address
Q16: Home Telphone

Pls share your experiences if anybody has done a parallel application in both Canada and Chennai consulates.
Pls help soon....

i think US address. if it says permanent address, then india's address.

Why I am asking specifically becuase: I think they are going to send the passport to the given addres if it is India. If we give US address where they will send?

This is a confusion for me.....
