Filing I-485 when I-140 is pending - Issues

My I-140 details are:
RD-05/13/2002
ND-05/15/2002
AD- <not approved yet>

Can I go ahead and file I-485 (EAD) ? If yes then what are the FORMS I need to fill up ? I just read somewhere that I may have to fill G-28, G-325 forms also in addition to I-485 app and Medical.
Is this true ?
My attorney has again sent me the Background info sheet that I'd sent while filing I-140 ! Isn't it unusal ?

I have not received any other forms, not even I-485 application, so far. Will Attorney send me these forms directly or will these come from my employer ?

Yes,

You can down load the whole package from INS site. But medical forms need to get from Doctor. Some do accept the downloaded forms and some don't.

Some hospitals can provide those forms, u need to find out that with them.

Yes, background information is required, but I am not sure how attorneys work on them, may they have questionire kind of thing
