Filing I-485, I-765 questions. Plz help

I have few questions while filling up I-485, I-765, I-131 applications.

1)What should I put where "Non Immigrant visa number" is asked in I-485 form?
I am on h1-b and wife on h4. Filing I-485 for wife as a primary (Schedule A) and myself as derivative.

2)In past both mine and wife's I-485 was denied due to underlaying I-140 denial. We both are maintaining h1, h4 status. Now For below question in I-485 form, should I just check on "Yes" and give date, place of filing and for final dispostion should I write "DENIED" or do I need to give any more explanation? I believe, no explanation as USCIS can find detail if they wish to, correct??
"Have you ever before applied for permanent resident status in US? No Yes checkboxes. If you checked "Yes" give date and place of filing and final dispositon."

3)Before previous I-485 denial. we got EADs and received SSN# for wife (who is on h4). I should write her SSN# in I-485 form right?

4)What should I write where A# is asked? A#, which was issued on previously denied I-485 receipt?

5)I remember submitting passport size pictures previously but today while I am filling up these forms. I don't see any places to stick pictures. Don't I need to submit 2 passport size pictures?

You just answer the question yourself.

Better to attach a letter to explain that. Don't assume USCIS knows everything.

Of course.

4)What should I write where A# is asked? A#, which was issued on previously denied I-485 receipt?

Yes. They could reuse it or give you a new one. Mine got a new one from the one they gave me for my OPT EAD.
