Filing for EAD - Requirements?

I have given up hope of getting 485 Approval in next 2 months and are looking to file for my 3rd EAD before it expires. So guys can you please tell me what all are needed to file for one. I know that there are websites telling what are required, but feeling lazy to read those again third time.

So my understanding is


Fill I-765
2 Photographs
I-485 Copy
I-94 Copy
Existing EAD copy


Please let me know if anything else is needed.
Also any idea how much time it's taking for renewal?
 
Why dont you e-file 765, you need not send any thing..., except, you need to take an appointment with the closed application support center for photograph and right hand index finger, fingerprint. Much better... than paper filing.

Is it faster when you do efile?

I thought about it, but don't want to go thru the pain of going to Center and all. I have H1 which is valid till next july. Just playng safe.

E-file is not any faster than paper file.

Actually for most folks, paper file is faster !!
