1. Part 1B says "Your name exactly as it appears on your Permanent Resident Card"
Full Middle name (if applicable)

My PR Card has my middle initial, not Full Middle Name.

Question: Should I give just my middle initial?

2. Part 6A asks dates of current home address.

Question: In the To column should I put date application mailed to the Service center or 'Present'.

3. Part 8A How many times have you been married?
Question: If currently married and it is my only marriage, write/type 1?

4. Part 10G33 Regarding Selective Service Registration
Are you a male who lived in the US at any time between 18th .......

Question: I did live in US before I was 26, but as a legal nonimmigrant. The question is somewhat misleading. Do I answer Yes or No?

Thank you.

look at the bottom of your green card.. it should print as LAST>>FIRTS>>MIDDLE.

I put the my full middle name on the form.

i put 'present'. if you put any date on the "To Column", you are reporting you only stay in you "current home address" till the "To-date". make sense?

i was on the same situation, I answered no. the question IS NOT misleading. it said ".... in any status EXCEPT as a lawful nonimmigrant?"

I have similar issue. On my passport, my original last name cut into 2 part(because it was too long). So, During GC process, my company lawyer thought 1st part was my middle name. So, my GC shows my first name and 2nd part of the last name. That eventually has become my last name now. so, I do have same question as you. :(

If I understand it correct, you should put date from which you are in your current resident. eg: 12/01/2003 - Present.

As far as I know (and learnt from this forum), you don't have to worry about Selective Service registration if you got you GC after 26 years and entered the country with non-immigrant visa.
