There is no process for an H1B transfer. You will need to apply for an H1B all over again, although you will not be counted against the quota, and can therefore apply even if there are no more H1Bs available from the annual quota. In your application, the only difference will be that you will mention that you are in the USA in H1B status, and will give some related information. But the forms, fees, etc. are exactly the same as a new H1B.
The fees are as follows:
1) $185 for the basic application - I-129
2) $750 or $1500 for H1B Data Collection, depending on the size of the company, and some other factors - read the documentation that comes with the I-129 form
3) $500 Fraud Detection Fee
4) If you want to use Premium Processing, another $1000. This process will get you a response within 15 calendar days.

Only your employer or the employer's representative can legally apply. If you are the beneficiary, you cannot apply on your own behalf, although if your employer wants your help, perhaps you can help - again, remember you are not supposed to do any work, paid or unpaid, excepting what your existing H1B is approved for.
You don't REALLY need a lawyer, but most employers don't have the skills, time, or inclination to apply. So you should go through a lawyer, or at least an employer who understands the process.
It is up to every lawyer, but I generally see rates going from $500 to $1500. Do some searching on the Web. You don't have to limit yourself to someone in your city - most Immigration attorneys will work for you from wherever they are in the USA.
