FAX Enquiry to CSC

Could anyone give the following details to send a fax
enquiry to california service center.

Basically this is regarding not receiving the receipt
for the EAD.

1) Fax Number

2) Details to write in the document.

Please do let me know if anyone has send the enquiry through fax and received the receipt.
