Factors or circumstances based on which INS can deny I-485

What are the factors or circumstances based on which INS can deny I-485 to an individual? I have seen people sharing thier experiences on approvals, RFE\'s & delays in processing of 485. Never heard abt anyone sharing experience on denial. I don\'t mean to discourage anyone on this forum and neither rub someone on the wrong side who has been denied AOS.
