F1 visa interview after F1-H1B-F1

Hello,

I have a question about F1 visa in Mumbai (or any other consulate in India). Here is a brief history:

I came to USA in 2003 on F1 visa for MS
After completing MS in May-2005, I was OPT for a month.
I was on a 2-year H1B visa from May-2006 thru 2008.
I went to India and got the H1B stamped in Sept-2006.
I applied for another H1B petition through the same company and was approved from May-2008 thru May-2010.
I joined school for PhD in July-2009 after applying for COS from H1B to F1.

Thus, I had a F1 visa stamped on my passport, which expired in July-2008 and an H1B visa stamp, which expired in Sept-2008. I do not have any valid visa stamped on the passport. Of course, I have always had approved petitions or I-20s that legalized my stay in the country.

Now, I am planning to go home for a visit and have to apply for F-1. I completed the DS-160, paid the visa fee and about to get a date for the visa interview. I would appreciate it if you have any suggestions, criticisms or corrections for me.

I am going to apply for a fresh F1 visa and not a renewal. Is that correct?
My residence is in Mumbai, Maharashtra. So is Mumbai the only consulate that I can appear for a visa interview? Am I allowed to go to other US consulates in India?
For Mumbai, I do not see any dates available in July. Is that normal? Is there a future time window for booking visa interview dates, i.e. you can only see 2 months in future and nothing more than that?

Are there any things, points, specifics that I should be aware of or keep in mind?
