F1 to B2 Status, then how to B2 again

Hi... I was on F1 visa. Finished the degree & changed the status to B2 visa. Right now I'm on B2. It is valid until March 20, this year. In the meantime I had some health issues. I've doctors letters. I want to apply again for B2. What form I need to apply? Last time it was a change of status from F1 to B2. I submitted I-539. This time again I'm applying for B2. So, what form should I fill out, if you could let me know.
 
Thank you so much. I've one more question. If the B2 gets rejected, how much time I'll get to leave the US?

When your Change of Status was approved, you got a new B2 I-94 as part of the page of the I-797A approval notice. That is your current I-94. The F1 I-94 you got from entry should be disregarded.

Like I said, you don't "get" any time. You are technically deportable immediately, but they are unlikely to seek you for deportation anytime soon.

If your health improves enough to travel before a decision is made, you should leave, and that way, you don't risk a denial while you are here.

You get no time. You are out of status when your current stay ends, but you don’t accrue unlawful presence if you’ve filed a change or extension of status. if it’s approved, that is backdated to when your current status ends. If it’s not approved, you remain out of status and need to leave immediately.

This USCIS guide explains it - see section bottom left of page 3 (obviously you can’t work on B status so that part doesn’t apply to you)
 
Yeah... I just read that part. Thanks for sharing this information. I am worried because I want to apply for ESL course if B2 gets rejected. It takes at least 15 /20 days for the procedure & get the I-20. I'm worried if I can get that time to enroll.

You sound like you speak English just fine, and you completed a degree on your F1 so presumably you don’t need ESL. My understanding is USCIS doesn’t take kindly to frivolous requests for change of status.
