Extraordinary Ability INS forms

I plan to file an I-140 petition (separately with I-485 filing) in the category of Extraordinary Ability. Can anyone tell me if there are other INS forms (ETA-750, G325, etc.) I would need to submit along with the I-140 form and supporting documents? Thanks a lot.
