Extension of visitor visa - How to go about it.

My mother got a 10 yr multiple entry visitor visa stamping. She came to US on Dec-10 2005 and got an I-94 valid till 1-June-2006. I want to apply for an extension of her stay(I-94 extension). Please tell me how I can go about doing this(What is the procedure?). What documents are needed and how much would it cost me.

Please tell me as I have very less time to start the process.
