Extension of B-2 Visitor Visa

My mother in law has a 10 year visitor visa entry permit to the U.S.
She first visited the country in 1999 for 6 months. Later on she visited in 2002 for 6 months during which we applied for another 6 month extension which was approved. This year she arrived in May, again on a 6 month visa. We plan on applying for extension. My questions are:

1) When should we apply for extension? Is there a waiting period before one can apply for the extension?

2) What are the reasons we can give in order to apply for extension? Can we cite reason as "coming for child's delivery" as a possible reason for extension?

3) Since we have already extended her visa once, is there a chance that her visa extension will be denied?

4) Can someone send us a sample letter citing reasons for extension?

DONT MENTION the Pregnancy as the cause for any Vistior's VISA issue,
A Visitor's Visa is not meant for helping the Pregnant Ladies. If by any reason, INS knows that this is the reason, for sure, they will ask ur MIL to go back IMMEDIATELY.
