Extended I-94 not taken at the airport

My sister applied for the extension on her B-1 stay and got it approved. I have read in this forum that she needed to surrender her new I-94 which came with the extension approval letter (The bottom portion of the approval letter) along with the expired I-94 stapled to her passport when leaving USA.

But today while she was leaving; the airline staff just took her old expired I-94 not the new approved one and when I repeatedly asked they said only the old one is needed.

At that point I called the local USCIS office at the airport and the officer suggested that she should infact hold on to her new I-94 and should not surrender it. I also met one USCIS agent at the airport and he said the same thing.

Now I am not sure if that was the right thing to do. Should she have deposited her new I-94? If yes, now that she has already left the country what are her options.

She did the right thing when she stapled the new I-94 onto the old one, but she doesn't have to be worried. Her departure should have been recorded correctly only with the I-94 isssued by a CBP officer at POE. I'm not sure if she should hold on to the new I-94 issued by USCIS because she is out of the country now and a new I-94 will be issued next time she enters the United States. But I don't think she needs to surrender the one she has right now either just to correct her departure record.

When she enters the United States next time, she shouldn't have any problem. Even if (That's a big if, I don't think it will happen) she goes to secondary processing, they should be able to confirm her last extension and its approval (therefore there has been no overstay).
