Experts - please help!

I'm currently on H1B with a pending employment based I485 (EB3 category). I married my wife (USC) a year and an half ago and we decided to file a family based I1485 petition. We are currently preparing the forms and we have a few questions.

1. I864 EZ: Is this a mandatory form for everyone filing I864EZ? Both of us have a long employment history and are currently employed and have no problems disclosing our incomes. We just want to make sure if this form is really needed.

2. I485: (a) Application Type Question: In my employment based I485, the option a. was selected for Application Type (An immigrant petition giving me an immediately available immigrant visa number that has been approved). What should we choose for this family based petition.
(b) Fees: Should it be one check for $1,010 or 2 checks of $930 and $80 for biometrics.

3. I765: (a) Applying For: When the company applied for employment based 485 few years ago, I got my EAD. I never used EAD as I always had my H1B. Hence, should I select "Permission to accept employment" or "Renewal of my permission to accept employment (attach previous employment authorization)"?
(b) Question 16: Eligibility under 8 CFR 274a.12 ( ) ( ) ( ). For my employment based application, it was (C) (9) ( ). What should it be for my family based application now?
(c) Fees: Is it true that there are no filing fees if this is filed with I485?

4. A #: In all forms displaying a A number, should I specify the number that I have from the employment based petition?

I'm just going to quickly answer some of the questions that I know the answer to off the top of my head.

1) Your spouse must be the primary sponsor for the affidavit of support, and for this will fill out the I-864.

2) a)I dont have the form in front of me, though you will check the option which relates to something along the lines of 'immediate relative' (i think). Actually, Ive just downloaded the form, and I would select (h) and then explain your eligibility.
b) Just include one check of $1010.

3) a) b) I dont know the answers to.
c) Yep, if filed with the I485, no additional fee is required.

4) If you already have an A number, then yes, include this in your petition where it is requested.
